Gov. Charlie Baker plans to file reform legislation and the possibility of legal action remains on the horizon after the release Wednesday of a scathing report outlining failures, errors, delays and "utterly baffling" decisions preceding the deaths of at least 76 veterans with COVID-19 at the Holyoke Soldiers' Home.
